We are a leading provider of "Independent Support Coordination" (ISC) services for persons enrolled in Tennessee's Home and Community-Based Medicaid Waivers. We have been at the forefront of the development and provision of ISC services in Tennessee for nearly 20 years. Since our inception in 1996 as the first ISC agency in the state, we've served over fourteen hundred persons with disabilities and their families in planning the kinds of lives they want with the supports available to them in the Medicaid Waiver.
We've also partnered over the years with the state and other stakeholders to plan, coordinate and support scores of persons in moving from state-run facilities to homes of their own in their chosen communities.
Our agency was founded on adopting and promoting national best practices in the field of intellectual disabilities in Tennessee. Since our inception, we have held to certain core principles inherent in "person-centered thinking and practice". These principles include:
Listening to and learning about what people want in their everyday lives;
Helping people take and maintain control of their lives;
Working together with a person and his/her family, friends and professionals to support the person in living the life that they want;
Emphasizing the person's connection and contribution to the community;
Recording information in a person-centered plan for the person;
Doing business that puts the person at the center of what we do; and
Giving and supporting hope for a good life.
